Are you the future policyholder, or are you applying for insurance for someone else?

Please make sure you tick the applicable option here. For example, you can take out international health insurance for your son or daughter as the policyhilder.

From when would you like to be insured?

The start date of your insurance cover can be either the day you sign the application (today) or a maximum of 4 months in the future. Here, it is necessary to specify the exact start date, according to the pattern: DD/MM/YYYY.

Please enter your personal data here.

Please make sure that your details exactly match the data on your identity card. Please also enclose a copy of your identity card or passport with your application.

In addition, please follow these instructions so that we can process your application as quickly as possible without any queries:

• Please give us a precise description of your professional activity and industry (e.g. Sales Manager in the insurance industry).
• Do not forget to include the international country code of your telephone numbers.
• Be sure to check your e-mail address for spelling mistakes. We will use your e-mail address to send you your insurance policy, your premium invoices and access to the MyGlobality customer portal, and to keep you informed of any changes to your benefits. We will only contact you for advertising purposes if you give us your express consent to do so (see the privacy policy section).
• If you have more than one nationality, please state each additional nationality separately. There may be legal consequences for your insurance relationship.

Why do we ask you for detailed information on your countries of residence?

In rare cases, your nationality and country of residence may have legal consequences for the conclusion of an insurance policy. We are therefore obliged to document this information.

• Current country of residence: The country in which you are staying at the time of application and in which the application was signed. (The country should therefore correspond to the place of signature on page 14).
• Future country of residence: The country where you will live as an expat. (Note: If you plan to stay in more than one country, please indicate a main country of residence. If you are planning a world or round trip, please indicate your first destination).

Please complete the data as already explained above, in the section on the policyholder (A).

Correspondance address

Please enter the address of the co-insured person here. This is only necessary if this address differs from the policyholder’s address given above.

Please tick here the destination region in which your country of stay is located.

Please note: If you choose destination region II (worldwide without USA), travel to the USA of up to 6 weeks is still covered. During this period, Globality Health will cover costs for accidents and medical emergencies.

Please do not forget to tick in which of the three possible currencies you would like to pay your insurance premiums.

Please provide us with some information about your previous insurance, as well as about your treating general practitioner. This will not only facilitate and speed up the processing of your insurance application, but also the processing of your submitted invoices and benefits during the insurance relationship. You can also specify a statutory or state insurer as your previous insurance.

Health insurance cover for the last 5 years

Please note the following information in particular:

• Full name of previous insurer(s) (e.g. Globality Health and not just Globality)
• Specification of the corresponding insurance number(s)
• Exact details of the insurance period (start and end)

If there has been no health insurance cover in the last 5 years, we need the contact details of  at least one general practitioner (point 2).

Attending physician (general practitioner)

The following information should be given special attention:

• Full name of the doctor who is familiar with your health condition and with your medical record.
• Make sure to give the correct address of the doctor, or the doctor’s practice (or hospital/clinic).
• In addition, please be sure to include a current telephone number as well as your doctor’s e-mail address. 

Of course, you can also indicate more than one doctor per insured person.

If you do not have a general practitioner who is familiar with your medical history, we need the contact details of your health insurance for the past 5 years (point 1).

Moratorium

Do you want to undergo a medical health check when you apply – or do you waive it and opt for a moratorium?

Please ensure that you tick the applicable option.

For which persons should the health questionnaire be completed?

Should you have opted for the full medical underwriting, please note that the questionnaire must be completed for each person to be insured.

What should I bear in mind when stating routine dental check-ups (question 11a)?

If you have only been to a dentist for routine check-ups in the last 5 years and no findings were found, please state this briefly under “Further information” (e.g. Routine check-up – no findings).

How can I provide more detailed information about my health?

If you answered „Yes“ to one or more of the questions in the health questionnaire, we will need further information on this, for example in the form of medical documentation or a medical report.

To speed up the process, please make sure that your information is as complete as possible: Fill out each field in the table and submit medical reports and expert opinions on them to us, if available.

If you have more than one insured person, please make sure that you provide information on the person in the first column.

If the space in this table is not sufficient to provide all the required information, you are also welcome to use the free area under “Further information and remarks”.

How often would you like to pay your premium?

To enable a smooth payment process, please let us know which of the four payment methods you prefer. the payment frequency has no influence on the amount of your premium.

What should I bear in mind regarding the payment method?

As described in the application, direct debit is unfortunately only possible under certain conditions.

If you decide to pay by direct debit, please complete the direct debit mandate on page 9 and submit it together with the application form.

If you choose to pay in British pounds (GBP) or U.S dollars (USD) ,we can unfortunately only accept bank transfers or credit card payments as a payment method at this time.

Payment method

We kindly ask you to agree to recurring payment. By doing so, you confirm that we may collect your insurance premium on a regular basis. With this confirmation, you not have to worry about anything else and avoid late payments or possible dunning procedures.

Signature of the payer

You can sign the insurance application form by hand and send it to Globality Health. Alternatively, we will accept a “facesmile signature” – for example, via a touchpad, or a digital signature with 2-factor authentication, such as DocuSign.

In order to process your application and data, we need several consent forms from you. A large number of these are mandatory in order to take out insurance with Globality Health.

These are the only two items where you have the option not to give your consent:

Medical secrecy lift

• If you give us your consent, this facilitates and accelerates the processing of your insurance application and also subsequent applications for benefits under your insurance. In this way, we can, for example, contact the attending physician directly in the event of a hospital stay and ensure that costs are covered quickly and the associated direct billing.
• You are also welcome to opt for the second option and refuse your consent to data processing. In this way, you retain full control of the transmission of your medical data. In this case, however, we are regularly dependent on your support and delays or even rejections of benefit claims may occur.

Direct marketing

Globality Health occasionally informs insured persons about new offers and additional benefits. In most cases, contact is made by email and is limited to a small amount of information. You have the choice whether you want to give us advertising consent for this. You also have the option to revoke your consent at any time.

Place and date

• Place and date: Please state the name of the place where you sign the insurance application. As described above, the location should match the country of residence on page 1. Please note: In rare cases, your country of residence may have legal consequences for the insurance policy.
• Since we are based in the EU, it is not possible for us to offer insurance in every country in the world.
• In other cases, we may be able to offer you insurance coverage for a specific country of stay, but the contract cannot be concluded if you are already in that country. However, if you make the application before your departure, the conclusion would be possible. Policyholder’s signature: The policyholder’s signature is mandatory, otherwise your application cannot be processed.
• Do you need further advice on this? Then contact us.

Signatures

• Policyholder’s signature: The policyholder’s signature is mandatory; otherwise your application cannot be processed.
• Insured person 1: A signature is only required if the policyholder is not also one of the persons to be insured.
• Insured person 2/3/4: These signatures are only required if there are several persons to be insured. In addition, only signatures of persons over 18 years of age are mandatory.
• Insurance intermediary and no.: The data and signature(s) of the intermediary(s) are mandatory (insofar it is an intermediary transaction), otherwise the application cannot be processed correctly.
