The start date of your insurance cover can be either the day you sign the application (today) or a maximum of 4 months in the future. Here, it is necessary to specify the exact start date, according to the pattern: DD/MM/YYYY.
Please make sure that your details exactly match the data on your identity card. Please also enclose a copy of your identity card or passport with your application.
In addition, please follow these instructions so that we can process your application as quickly as possible without any queries:
In rare cases, your nationality and country of residence may have legal consequences for the conclusion of an insurance policy. We are therefore obliged to document this information.
Please complete the data as already explained above, in the section on the policyholder (A).
Please enter the address of the co-insured person here. This is only necessary if this address differs from the policyholder’s address given above.
Please tick here the destination region in which your country of stay is located.
Please note: If you choose destination region II (worldwide without USA), travel to the USA of up to 6 weeks is still covered. During this period, Globality Health will cover costs for accidents and medical emergencies.
Please do not forget to tick in which of the three possible currencies you would like to pay your insurance premiums.
Please provide us with some information about your previous insurance, as well as about your treating general practitioner. This will not only facilitate and speed up the processing of your insurance application, but also the processing of your submitted invoices and benefits during the insurance relationship. You can also specify a statutory or state insurer as your previous insurance.
Please note the following information in particular:
If there has been no health insurance cover in the last 5 years, we need the contact details of at least one general practitioner (point 2).

Of course, you can also indicate more than one doctor per insured person.
If you do not have a general practitioner who is familiar with your medical history, we need the contact details of your health insurance for the past 5 years (point 1).
Do you want to undergo a medical health check when you apply – or do you waive it and opt for a moratorium?
Please ensure that you tick the applicable option.
Should you have opted for the full medical underwriting, please note that the questionnaire must be completed for each person to be insured.
If you have only been to a dentist for routine check-ups in the last 5 years and no findings were found, please state this briefly under “Further information” (e.g. Routine check-up – no findings).
If you answered „Yes“ to one or more of the questions in the health questionnaire, we will need further information on this, for example in the form of medical documentation or a medical report.
To speed up the process, please make sure that your information is as complete as possible: Fill out each field in the table and submit medical reports and expert opinions on them to us, if available.
If you have more than one insured person, please make sure that you provide information on the person in the first column.

To enable a smooth payment process, please let us know which of the four payment methods you prefer. the payment frequency has no influence on the amount of your premium.
As described in the application, direct debit is unfortunately only possible under certain conditions.
If you decide to pay by direct debit, please complete the direct debit mandate on page 9 and submit it together with the application form.
If you choose to pay in British pounds (GBP) or U.S dollars (USD) ,we can unfortunately only accept bank transfers or credit card payments as a payment method at this time.
We kindly ask you to agree to recurring payment. By doing so, you confirm that we may collect your insurance premium on a regular basis. With this confirmation, you not have to worry about anything else and avoid late payments or possible dunning procedures.
You can sign the insurance application form by hand and send it to Globality Health. Alternatively, we will accept a “facesmile signature” – for example, via a touchpad, or a digital signature with 2-factor authentication, such as DocuSign.
In order to process your application and data, we need several consent forms from you. A large number of these are mandatory in order to take out insurance with Globality Health.

Globality Health occasionally informs insured persons about new offers and additional benefits. In most cases, contact is made by email and is limited to a small amount of information. You have the choice whether you want to give us advertising consent for this. You also have the option to revoke your consent at any time.
